By submitting this form, you authorize Simply World Class and/or Perfect Reflections Travel, to debit/charge your card for travel services and you agree to the terms and conditions of the Supplier(s) who will be providing your travel services. Your signature is required to complete your travel plans. Please read the cancellation/change penalties that may apply to changes or cancellations of your travel.
A completed registration and debit/credit card authorization form is required for Simply World Class and/or Perfect Reflections Travel to process your flexible monthly payments (if applicable/requested). You will be required to complete an updated registration and debit/credit card authorization if your debit or credit card details change. This will help to avoid confusion and relieve any issues just in case you may need to switch cards, inform us of any changes to your trip account (additions/removals of roommates, changes to travel dates, etc., or your advanced/monthly payment/ final balance amount differs from the previous month(s). Please submit to avoid late fees and/or penalties. Thank you.
FLEXIBLE PAYMENTS (REQUEST ONLY):
After the initial deposit per person, registrants requesting flexible monthly payments will be required to make subsequent on-time monthly payments per person which will be determined by Simply World Class and Perfect Reflections Travel, and divided out based on the total of your package which is due on a specific date of every month (based on your deposit received date) or late fees and/or cancellations may incur.
Once your online registration/authorization form has been completed and submitted, you will receive a confirmation message via the Simply World Class website which provides Simply World Class and Perfect Reflections Travel authorization to initiate, set up and finalize the invoicing process to allow you to make monthly payments. When making deposits or payments, you will receive an e-mail confirmation from the vendor supplier which will provide you payments received, balance history, and reminders of final balance due. With our supplier, in order to add travel protection after you have made your reservation, call Simply World Class at 617-293-0793 or email Kim Woodruff at SWC@simplyworldclass.com. Travel Protection Plus if offered through our supplier, and may only be added within seven (7) days of your initial deposit, provided you have not yet made your final payment. You must have travel protection with our supplier in order to receive a full refund via your original payment method. If you have to cancel prior to the cancellation policy date and do not have travel insurance with our supplier, you will only receive a full credit which expires one (1) year from the original date of booking, and you must book and travel by this date.
Travel insurance is additional per person (Land-Only Portion) which will be reflected in the final total of your travel package as an add-on if you choose to accept. Travel insurance cost is based on your age, the state you live in, and the total booking travel package cost per person (including flights and transportation).

With the supplier, your insurance cost will be included in your total trip package cost where you will be allowed to make flexible payments.
